Access forms on the Coast Colleges District website.
Anyone who needs to sign the Photography and Video Image Release Form must sign up for an account with Dynamic Forms in order to verify identity via email address.
If participant has a Coastline account, use the Single Sign On Image Release Form.
If participant does not have a Coastline login, use the Non-Single Sign On Image Release Form.
If participant is a minor, they will need to include a parent/guardian's email. Once minor has filled out the form, dynamic forms will send to the parent's email for signature. (May also want to notify ahead of time so that email is not mistaken for spam.)
Maxient is the software tool that Coastline uses to manage student behavior records at the college. It is a centralized reporting and recordkeeping system for student discipline, academic integrity, care and concern records, and Title IX matters. Please use this Maxient Incident Reporting form to submit allegations related to any of the following:

Submit an IT Service Desk request:
Faculty and staff may submit requests online via the District Service Desk website.
Submit a request for service from Maintenance and Operations department.
Submit a request for marketing, promotions, branded items, and website page updates and/or training to the Marketing Director via email. Coastline logos can be found on the branding page.
Submit a request for business cards, brochures, fliers, posters, and more.
Access the Coastline PDI Conference and Workshop Funds Application and the CCCD Conference Authorization Request form for faculty. Also access all other PDI forms.
Staff and faculty can refer students to Student Mental Health Services using the ICare Request form and a therapist will reach out to the student. Please read the whole form carefully before submitting.
Non-Banner courses can be requested by completing the request form which will be forwarded to your Vice President for approval.
