
How to Apply and Eligibility
Eligibility
Students with verifiable disabilities that affect their participation in the learning environment are eligible and encouraged to apply for academic accommodations and support services through the Special Programs and Services for Students with Disabilities office. In order to receive services, students must complete an application and provide documentation of a disability from a medical professional/agency that clearly states the diagnosis. Examples of documentation that may verify a student’s disability include, but are not limited to:
- Medical report
- Psychological report
- Individualized Education Program (IEP)
- 504 Plan
- Veteran’s Administration (VA) Disability Rating report
- Department of Rehabilitation records
- Regional Center records
Examples of different types of disabilities include, but are not limited to:
- Acquired Brain Injury
- Attention Deficit-Hyperactivity Disorder (ADHD)
- Autism
- Deaf or Hard of Hearing
- Intellectual Disability
- Learning Disability (dyslexia, dyscalculia, dysgraphia, etc.)
- Mental Health Disability (depression, anxiety, schizophrenia, schizoaffective disorder, bipolar disorder, personality disorders, etc.)
- Physical Disability (physical condition which affects mobility/movement, spinal cord injury, paralysis, amputee, broken, sprained, or fractured limb)
- Visual Impairment (blind or low vision)
- Other Health Conditions and Disabilities (epilepsy, cancer, diabetes, migraines, end-stage kidney failure, multiple sclerosis, lupus, HIV, narcolepsy, etc.)
Application Steps
- If you are new to Coastline College, apply for admission. You will receive a student ID number via email.
- Complete a Special Programs and Services for Students with Disabilities Application.
- Upload your verification of disability paperwork to the application. If you do not have access to paperwork which verifies your disability, you can complete an authorization to release records form which will allow our office to request this information from your provider or appropriate agency.
- Once our office receives your application and verification of disability paperwork, we will contact you to schedule an intake appointment with one of our counselors.
During the intake appointment, you will discuss your goals, courses, accommodations, and your rights and responsibilities as a student in our program.
