Instructions: You may (1) print this form, then fill it out by hand; or (2) fill out the form online, then print it. Either way, the student and the foreign student advisor must sign their names where indicated. Directions for submitting this form by mail are near the bottom of the page.

To the Student (Applicant)

All students who are currently in the United States on an F-1 visa and wish to transfer to Coastline Community College must have this form signed, dated and completed by the International Student Advisor or Designated School Official. You are responsible for having this form completed and returned to Coastline no later than the specific deadline stated on your acceptance letter. It is important to complete your transfer in a timely manner in order to maintain your F-1 status.
